Pharmacy VAT and NHS accounts: getting the records right

A pharmacy can receive money from NHS dispensing, clinical services, private work and retail sales. Clear records help an accountant distinguish those activities, reconcile receipts and review VAT treatment. VMK helps community pharmacy owners build that financial picture without treating every receipt as the same kind of sale.

Separate income streams Reconcile NHS schedules and receipts Review VAT coding and purchase evidence Document the accounting decisions

Zero-rated, exempt and standard-rated are not interchangeable

Prescribed medicines can qualify for zero rating when the conditions are met, while over-the-counter medicine sales are generally standard-rated. Some qualifying professional healthcare services can be exempt. The actual supply and contractual arrangements determine treatment; a payment from the NHS is not automatically exempt.

Zero-rated supplies are taxable supplies at 0%, unlike exempt supplies. That distinction matters when reviewing VAT recovery. We can examine sales categories and invoices against HMRC VAT Notice 701/57 and document the conclusions for your records.

Partial exemption and input VAT recovery

Where a VAT-registered business incurs costs related to exempt supplies, partial-exemption rules can affect the amount of input VAT recovered. Mixed-use costs may need allocation and a documented calculation, including any required annual adjustment. Zero-rated sales and exempt sales must not be combined merely because neither shows VAT charged to the customer.

We can review how purchase costs are coded, the method being used and the evidence supporting it. We do not promise full VAT recovery. See HMRC's partial-exemption overview for the principles, then discuss your pharmacy's actual supplies and costs.

A monthly NHS pharmacy reconciliation workflow

For English community pharmacies, the NHSBSA Information Services Portal makes the FP34 Schedule of Payments and a Prescription Item Report available to authorised users. These are financial source records, not a substitute for the pharmacy's own sales and claims information.

    1. Collect the period's records. Identify the dispensing or service period, relevant schedule, bank receipt and supporting claims summary. Keep a clear reference to each source.
    2. Reconcile the movement. Explain differences between recorded income and cash received, including advances, adjustments, deductions and amounts relating to another period where applicable.
    3. Separate the activity. Record NHS dispensing, service income, private work and retail takings in appropriate categories. Review supporting documents before selecting VAT codes.
    4. Investigate exceptions. Maintain a list of differences to raise with the appropriate payment body or professional adviser rather than silently posting them to miscellaneous income.
    5. Close and review. Reconcile bank and supplier balances, record outstanding items and produce the agreed management information and cash-flow view.

Bring your NHSBSA payment records. Community Pharmacy England also publishes an explanation of the FP34 schedule. These resources help identify the documents to discuss; payment rates and claim eligibility must be checked against current arrangements.

Pharmacy First, private services and retail activity

Keep a recognisable trail for service activity, submitted claims, expected payments and receipts, including Pharmacy First where relevant. Do not apply a blanket VAT code merely because different services appear on one schedule.

We can help design a monthly records checklist that brings together service reports, retail/EPOS summaries, refunds, card settlements and bank entries. Responsibility for clinical eligibility, contractual compliance and timely claims remains a separate matter for the pharmacy and its relevant advisers.

Stock, wholesaler statements and cash flow

Capture supplier invoices and credit notes consistently and reconcile wholesaler statements to the ledger. Agree a year-end stock-count process and identify expired, damaged or obsolete stock for review. A purchase invoice and a payment are separate pieces of evidence, so bank reconciliation alone is not the whole bookkeeping process.

For management reporting, we can discuss income mix, stock costs, gross margin, staffing and overheads. A cash-flow forecast should distinguish expected income from available cash and make tax, wages and supplier payment dates visible. Forecasts depend on the information and assumptions supplied, not guaranteed receipts or funding.

Ask for a pharmacy records and VAT review

Tell us your business structure, VAT registration status, number of sites, accounting software and the issue you want resolved. We will agree the scope, records required and a quote before work begins. Share business records securely after onboarding; do not upload patient-identifiable dispensing records through the public enquiry form.

Discuss your pharmacy's bookkeeping and VAT, or return to our pharmacy accountant service page. Individual pharmacists should use the locum pharmacist route.

Service information prepared on 23 October 2026. Tax treatment depends on your contracts and circumstances. Our NHS payment guidance concerns community pharmacies in England; arrangements elsewhere in the UK need a separate review. Accounting support is not clinical, GPhC regulatory, legal or investment advice.

    Frequently asked questions

Is a zero-rated pharmacy sale the same as an exempt sale?+

No. Zero-rated sales are taxable at 0%; exempt sales are a different category. The distinction can affect input VAT recovery and should be reflected in the records.

What NHS records should I show my pharmacy accountant?+

Relevant FP34 payment schedules, available Prescription Item Reports, claims summaries, adjustments and bank receipts provide a useful starting point for English community pharmacy reconciliation. Agree a secure way to share the appropriate records.

Can you reconcile Pharmacy First payments?+

VMK can help organise the financial records for Pharmacy First and compare relevant schedules with the books and bank receipts. That work does not certify clinical eligibility or replace the pharmacy's claims process.

Can a pharmacy reclaim all its purchase VAT?+

Not automatically. Recovery depends on the use of the cost, supporting evidence, blocked-input-tax rules and any partial-exemption restrictions. We review your position rather than promise full recovery.

Does this NHS payment guidance apply everywhere in the UK?+

The NHSBSA pharmacy-payment examples here relate to England. Pharmacies elsewhere in the UK need their own payment-body and contractual arrangements reviewed.
